Rebate is limited during promotion period to one (1) offer per contact lens fitting exam, up to two (2) offers per household, up to $50, while supplies last. Group or organization requests are void. Rebates will be sent only to street or rural route addresses. Requests from P.O. boxes will not be honored. Original mail-in certificate, contact lens fitting exam receipt and sales receipt are required; reproductions are not acceptable. Valid on contact lens purchases and fitting exams between March 1, 2005 and August 31, 2005. All rebate requests must be submitted by September 30, 2005. No late submissions will be accepted. Please allow 6-8 weeks for processing. Offer only good in the USA and Puerto Rico. Void where prohibited by law.
